Terms and Conditions of DP Derm Care Plan - Accident & Theft
1. Plan Overview
The Dp Derm Care Plan provides extended protection for your Dermapen 4 device. The plan is available as a one-time purchase of $2,000 and provides coverage for a period of five (5) years from the date of purchase.
The Care Plan is designed to protect your investment by providing replacement coverage if your registered device is lost, stolen, or damaged during the coverage period.
2. Coverage Period
Coverage under the Dp Derm Care Plan begins on the date of Care Plan purchase and remains valid for five (5) consecutive years.
The Care Plan automatically expires at the end of the five-year term and does not renew automatically.
3. Device Eligibility
The Care Plan applies only to the specific Dermapen 4 device registered at the time of purchase.
The Care Plan:

Is not transferable to another device

Is not transferable to another individual, clinic, or business
-
Must be associated with a valid device serial number
4. Replacement Coverage
The Dp Derm Care Plan provides complete coverage for the registered Dermapen 4 device if the device is lost, stolen, or damaged during the five (5) year coverage period.
Under the Care Plan, the customer is entitled to one (1) replacement device during the five (5) year coverage period.
If the registered device is lost, stolen, or damaged and a claim is submitted and approved by Dp Derm, a replacement Dermapen 4 device will be issued.
Once a replacement device has been issued, the replacement benefit under the Care Plan is considered fulfilled, and no additional replacement devices will be provided during the remainder of the five-year coverage period.
The Care Plan itself will remain active for the remainder of the five-year term; however, no further replacement claims will be eligible once the replacement benefit has been used.
5. Claim Process
To submit a claim under the Care Plan, the customer must:

Notify Dp Derm Customer Support of the incident.

Provide proof of device ownership and Care Plan purchase.

Provide supporting information describing the incident, including photos or documentation when applicable.

Provide a police report in the case of theft, if requested.
Dp Derm reserves the right to request additional information necessary to verify the claim.
6. Claim Evaluation
All claims submitted under the Care Plan are subject to review and approval by Dp Derm.
If the claim is approved, a replacement device will be issued and shipped to the registered clinic or purchaser.
Dp Derm reserves the right to determine the appropriate replacement device, which may include a new or equivalent refurbished unit.
7. Device Recovery After Replacement
If a device reported as lost or stolen is later recovered after a replacement device has been issued, the recovered device must be returned to Dp Derm within fourteen (14) days of recovery.
Failure to return the recovered device may result in the customer being charged the full replacement value of the device.
8. Misrepresentation or Fraud
Any claim submitted under the Care Plan that involves misrepresentation, intentional damage, fraudulent activity, or misuse of the program may result in:

Immediate denial of the claim

Termination of the Care Plan

Recovery of replacement devices or associated costs
Dp Derm reserves the right to take appropriate legal or financial action in cases of fraud.
9. Exclusions
The Care Plan does not cover:

Loss or damage to accessories, chargers, batteries, or cartridges

Claims submitted after the Care Plan expiration date
10. Refund Policy
The Dp Derm Care Plan is a one-time purchase of $2,000 and is non-refundable once activated.
The Care Plan cannot be transferred, resold, or reassigned.
11. Modification of Terms
Dp Derm reserves the right to update or modify these Terms and Conditions at any time.
Any changes will apply to future Care Plan purchases and will be communicated where required.
